Application & Waiver

We offer two ways to send us your membership application:

The Fast Way

  1. Download the FREE Adobe Acrobat Reader  (Note: Other PDF readers may have mixed results and not allow a digital signature.)
  2. Download and save the waiver to your computer. Get the form here. (Do not fill it out in your browser as the information will not be saved)
  3. Open the waiver with Adobe Acrobat Reader and fill out the form.
  4. Digitally sign at the bottom.  When you click on the signature field, Acrobat will take you through the steps of creating the signature and saving the document.
  5. Save it with your last name and first initial in the file name (Pinkerton_R_CKC2016_Waiver.pdf)
  6. Email the waiver/s back to cantonkayak@gmail.com
  7. Pay for your membership using the PayPal button on the Join or Renew page.

The Slow Way

  1. Print the form. Get the form here.
  2. Fill it out.
  3. Send it by mail, along with a check for your membership fee, to the address on the form.  (Or bring your completed application and check to a CKC membership event and hand it to us in person!)
  4. Pay for your membership using the PayPal button on the Join or Renew page.

If you are a new member, you will be contacted to schedule your training class after your payment and application/waiver have been received.
